About Dom: 

Dom is a vibrant 60-year-old who experienced a significant health crisis in September 2023, leading to his lengthy hospitalisation. Following investigation, Dom was diagnosed with early onset dementia that required complex discharge planning and rehabilitation support. Once known for his active lifestyle and independence, Dom’s condition progressed to a point where his previous living arrangement was no longer feasible. Supported by his family, the focus shifted to helping Dom leave hospital by finding suitable NDIS-funded Medium Term Accommodation (MTA) that could bridge the gap to his long-term Supported Independent Living (SIL). 

NDIS Plan Goals for Dom: 

Dom’s referral was initiated by his team at Monash Health through our streamlined NDIS MTA Rapid Request process. Our dedicated Care Navigator team worked closely with his healthcare providers and family to evaluate his needs and identify a suitable residence to facilitate his transition from hospital. Our collective goal was to provide an environment conducive to Dom’s recovery and well-being while awaiting his SIL arrangement. 

Dom’s primary goal within his NDIS plan was to maintain as much cognitive and physical independence as possible. Additionally, to support his mental wellbeing, he sought to continue engaging in his interests, such as listening to music, shopping and spending time at the beach with a coffee. 

Finding the Right Fit: 

Drawing on her extensive experience in hospital discharge planning, our Lead Care Navigator, Jayde identified our property at Carrum Downs as the perfect fit to support Dom’s goals and NDIS MTA. Carrum is a spacious ground floor property with private bedrooms and accessible roll-in ensuites. Dom was encouraged to bring in familiar items from home to provide comfort and ease his transition. The bright, open communal living areas encourage social interaction and engagement with fellow residents. 

Collaborative Care: 

To ensure the highest level of support for our residents, we collaborate with our trusted onsite care providers, SAI Home Care. Led by Nicole Armstrong, the team at SAI are integral to our goal of providing personalised, high-quality care to MediStays guests. SAI’s dedicated team of experienced Support Workers are there 24/7 to support Dom in achieving his goals and promoting his independence out of hospital. In collaboration with MediStays, the SAI team also works alongside Dom’s family to gradually reintegrate him into community activities, ensuring he continues to engage in the hobbies he loves. 

Reflecting on Dom’s Progress: 

The supportive environment at our Carrum Downs property has significantly contributed to Dom’s recovery. His family and therapist’s regular visits ensure continuity in his care, allowing Dom to regain some independence and functioning in a peaceful setting as he awaits his SIL transition.
