At MediStays, we believe everyone deserves the chance to live independently. That is why we created the “Pathways to Independence” program.

This initiative supports NDIS participants who are living at home with their parents or in a shared home with support workers. Our goal is to empower participants to achieve their NDIS plan goals and live independently by providing the right accommodation, care and resources.

Understanding the Pathways to Independence Program

MediStays designed the Pathways to Independence program to support participants of the NDIS on their journey toward independent living. Whether you’re living with family or in a shared home with others, our program can help by offering the accommodation, care and information you need to succeed.

Accommodation and Independence: Our program provides the opportunity to take your first steps toward independent living in fully self-contained accommodation. This allows you to take supported steps towards managing your own space and responsibilities. You can experience the freedom of living on your own while having access to NDIS Support Workers and essential resources like meal planning.

Flexibility with Supports: MediStays provides accommodation and essential living information, with flexibility for your support workers. We offer full flexibility for your own trusted NDIS support workers to stay with you. Alternatively, request the assistance of MediStays partner organisations to provide your supports. We ensure you receive the assistance you need while you gain independence. The choice is yours!

How to Apply for MediStays Pathways to Independence Program

Applying for the Pathways to Independence program is easy! We have streamlined the process so you can start as soon as possible. Follow these simple steps to apply:

  1. Complete Our Online Form: Complete the online application form with your details, and make sure to state “Pathways to Independence” in the comments section at the end. Providing accurate information helps us understand your needs and tailor the program to suit you.
  2. Submit Supporting Documents: We will contact you to submit supporting documents, including your NDIS plan goals, plan manager information, and any relevant accessibility requirements. This information helps us assess your eligibility for the program.
  3. Await Confirmation: After submitting your application, it will be reviewed by our Care Navigators. We aim to respond quickly to all applicants. You will receive confirmation of your acceptance into the program and details about the next steps via email.
  4. Start Your Journey: Once accepted, you can start your journey to independence with a personalised plan designed to help you achieve your NDIS plan goals.

What You Can Expect from the Program

The Pathways to Independence program offers NDIS participants an essential opportunity to experience living independently. Our Care Navigators have extensive experience and are here to guide you every step of the way.

Here’s what you can expect:

  • Personalised Accommodation Planning: Our Care Navigators provide one-on-one support tailored to your specific needs. We work closely with you to ensure you receive the right accommodation placement and necessary information for living independently.
  • Five Short Stays: To boost your independence and capacity building, the Pathways to Independence program consists of a four-night stay, every month for five months.
  • Self-Contained Accommodation: Experience living in fully self-contained accommodation from one of our Pathways to Independence properties. This setup allows you to manage your own space and responsibilities, offering you a taste of independent living.
  • Nutritious Meal Support: Our meal planners help you maintain a healthy and balanced lifestyle.
  • Community Engagement: We encourage you to explore and participate in your local community. Depending on your individual preferences, you will have opportunities to engage with your neighbourhood and develop a sense of belonging.
  • Confidence: How can we assist you to build your skills to live independently? Why not invite a friend to participate in the program too? We can arrange for you to stay in accommodation together or in adjacent rooms.
  • Ongoing Evaluation: We evaluate your progress to ensure you’re on the right track. Our team is committed to helping you achieve your goals.
